We are looking for a systematic, analytical, and collaborative pharmacist to join us in a Drug Use Evaluation & Pharmacy Systems role. This is an exciting opportunity to join a patient-centered organization committed to excellence in healthcare.

POSITION OBJECTIVE:
As a member of the Niagara Health Pharmacy Team, the primary role of the Pharmacist, DUE & Pharmacy Systems is to promote the safe, effective and efficient organizational use of medications through monitoring and evaluation of medication therapy. Through the collection, analysis, and dissemination of medication-use data, DUE services guide the development and maintenance of hospital medication formulary, policy and best practice. DUE services help to guide and inform key internal and external groups such as practice leaders, clinicians, committees, and administrators. The Pharmacist, DUE & PS also leads the integration of sound clinical use of medications with operational management of medication supply chain and related information. This includes providing oversight to pharmacy purchasing activities, including medication procurement, inventory control processes, and contracting; including strategies to maintain supplies of needed pharmaceuticals during shortages and recalls minimizing their impact on direct patient care. Travel between sites is required.

KEY RESPONSIBILITIES:
o Support the review and maintenance of hospital medication formulary and related systems
o Participating in and guiding/leading medication-related contracting activities
o Providing clinical leadership and guidance on drug shortage evaluation and response
o Monitor drug use data and associated KPIs for insights and improvements
o Help organizational leaders understand, interpret, and improve the prescribing, administration, and use of medications
o Use financial reporting and analysis to guide data driven decisions and support drug budget adherence
o Collaborate with physicians, clinical leaders, and departments to enhance medication-related patient care and clinical outcomes, promoting safe, cost-effective medication-related care
o Lead Pharmacy and Therapeutics Committee activities, Chair the Formulary Committee
o Develop and help inform drug policies and associated information
o Build and maintain relationships with internal and external partners
o Understand and provide advice on how DUE decisions and priorities integrate within the functional guidance and workflows of the HIS
o Other responsibilities as assigned

QUALIFICATIONS:
o BSc Pharmacy or PharmD degree
o Current registration with the Ontario College of Pharmacists in part A of the register
o Hospital pharmacy residency required; or related clinical experience considered
o Minimum of 2 years of DUE-related experience, or equivalent combination of education/training and experience
o Minimum of 4 years continuous full-time previous hospital pharmacy experience required (within the last 4-6 years)
o Experience with critical appraisal and evaluation of clinical research and systematic reviews
o Experience working collaboratively with medical, pharmacy, and nursing staff
o Demonstrated understanding and proficiency in NAPRA Professional Competencies, CSHP standards, guidelines, OCP Standards of Practice and Accreditation Canada Medication Management Standards required
o Demonstrated knowledge of the principles and practices of drug use quality improvement and evaluation
o Knowledge and application of formulary management processes
o Demonstrated ability to foster a positive team environment, encouraging participation, respect, and cooperation to achieve desired outcomes
o Demonstrated experience in drug budget development, drug use fiscal planning, and management, with a proven ability to monitor and control expenditures in line with organizational goals and priorities
o Ability to manage and respond effectively to multiple simultaneous demands from key partners
o Strong analytical skills to interpret data and guide decisions supporting the delivery of safe, effective, efficient, innovative, and high-quality services
o Excellent organizational, interpersonal, communication, leadership, judgment, critical thinking, and decision-making skills
o Proven analytical, problem-solving, and troubleshooting abilities
o Ability to work effectively within a multidisciplinary team and in a unionized environment
o Proficient working knowledge of Microsoft Office programs (Word, Excel, PowerPoint, Outlook)
