LOCATION: MAROTTA FAMILY HOSPITAL - ST CATHARINES SITE
HOURS OF WORK: 75 HOURS PER PAY PERIOD

WAGE RANGE: $44.67 to $57.90 PER HOUR

POSITION OBJECTIVE:
As a member of the Wellness Recovery Integrated Comprehensive Care (WRICC) program, this position involves supporting adult individuals with complex, compromised, acute mental health and/or addiction issues. The successful candidate will have experience completing individual assessments, short-term psychotherapy interventions, and case management. Using a variety of evidence-based treatment modalities (CBT, DBT, Trauma-Informed, MI, etc.), the successful candidate will be skilled in engaging and providing integrated clinical best practice supports to Niagara Health adult clients known as 'familiar faces'. Having identified unique and vulnerable populations, the successful candidate will use the existing Illness Management and Recovery and various therapy modalities to collaboratively develop a therapeutic Health Link's Coordinated Care plan, which responds to the needs of the adult clients. The successful candidate will work effectively both on an independent basis and also with the inter-professional team and with community partners. The successful candidate's home site will be the St. Catharines Site; however, travel between sites of Niagara Health will be required, as well as working in the community in client's homes. Evenings may be required.

QUALIFICATIONS:
o Masters Degree in Social Work (MSW) in good standing required
o Registered with the College of Social Workers required with demonstrated knowledge of the Ontario College of Social Workers and Social Service Workers standards of practice required
o A minimum of five years' experience providing evidence-based individual therapy/treatment in an adult mental health setting required
o Education in evidence-based treatment modalities (i.e. Trauma Therapy, Cognitive Behavioural Therapy, Dialectical Behavioural Therapy, Solution Focus Therapy, and/or Motivational Interviewing) required
o Demonstrated knowledge of major mental health illnesses and main classifications of psychotropic medications required
o Demonstrated excellent clinical and case management abilities working with individuals living with severe mental health, trauma, concurrent disorders and with dual diagnoses
o Demonstrated experience facilitating team, community and care planning meetings
o Working knowledge of the Health Links Coordinated Care Plan model
o Working knowledge of Illness Management and Recovery (IMR) model and content
o Applies person-centered care and the recovery model of care in clinical practice
o Demonstrated organizational, problem-solving, critical thinking, decision making, conflict resolution and ability to establish clear professional boundaries
o Demonstrated ability to engage clients living with mental health/addictions issues
o Demonstrated ability to conduct comprehensive mental health assessments, provide therapeutic support and/or referrals to other services
o Demonstrated ability to collaborate in and/or formulate care/treatment/discharge plans while assisting clients through this process
o Working knowledge of formal and informal community services / resources within the Niagara Region which support clients with mental health/addictions
o Demonstrated ability to foster collaborative working relationships, using excellent interpersonal and communication skills, within Niagara Health and with Niagara Regions' community services/resources
o Demonstrated knowledge of relevant legislation including the Mental Health Act, Children and Family Services Act, Health Care and Consent Act and Personal Health Information Protection Act (PHIPA)
o Demonstrated commitment to professional development, ongoing education and integration of research into clinical practice
o Current Safe Management Group or Non-Violent Crisis Intervention training
o Basic proficiency in computer skills and within Microsoft Word applications
o Applies Niagara Health CORE values, professional practice standards, and recovery/client-centered philosophy to client care and services
o Consistent and regular attendance with the flexibility to adapt working hours (including evening hours) to support client needs
