WAGE RANGE: $48.21 TO $64.50 PER HOUR

POSITION OBJECTIVE:
The AS Pharmacist is an integral member of both the Pharmacy and Antimicrobial Stewardship Teams, providing direct patient care as well as contributing to the antimicrobial stewardship strategy at NH. This full time-position is suited to a self-directed individual with an interest in optimizing patient pharmacotherapy to improve patient outcomes and antimicrobial utilization. In this position, you will apply clinical knowledge and patient assessment to support prospective antimicrobial use audit and feedback, and facilitate appropriate prescribing of antimicrobial therapy. As a member of the organizational AS team, you will also work on broader initiatives that foster improved antimicrobial use, lower resistance, and improve patient safety such as formulary reviews, education, development of NH-specific reference materials, and collection and analysis of data.

QUALIFICATIONS:
o BSc Pharmacy or PharmD degree
o Current registration with the Ontario College of Pharmacists (Part A) required
o Completion of hospital pharmacy residency or specialty AS program (certificate or other) required
o Minimum of 3 years of experience in an acute care hospital pharmacy required
o Demonstrated clinical patient assessment, pharmaceutical care and clinical documentation skills
o Strong leadership, critical thinking, problem-solving and decision-making skills
o Excellent interpersonal, organizational and communication skills
o Ability to establish effective working relationship with others across disciplines and departments
o Ability to work independently with minimal supervision
o Demonstrate self-directed learning and continuing education
o Positive attitude, flexibility to adapt to changing organizational needs and accountability
o Evidence of professional development and continuing education. Excellent communication and interpersonal skills
o Demonstrated ability to communicate and work effectively within a multi-disciplinary team
o Consistent and regular attendance required
