WAGE RANGE: $48.210 TO $64.500 per hour
POSITION OBJECTIVE:
This Full-Time position is suited to a flexible, self-directed individual who enjoys leading improvement and change. The successful candidate will possess detailed knowledge and have practice experience in medication safety, process improvement, root cause analysis, and hospital pharmacy operations and medication use systems, including pharmacy automation and hospital information systems (HIS).
You will serve as an expert resource on medication safety for the pharmacy program and will collaborate with other programs in support of a safer medication-use system. You will identify best practices for medication safety and lead medication associated change and improvement efforts, to include those associated with pharmacy automation and HIS.
RESPONSIBILITIES:
o Optimizing medication use: guiding the design, implementation, and maintenance of safe medication systems, including automation and technology
o Providing direct expert insight and improvement strategies during quality and safety reviews; leading any resulting pharmacy-related changes and supporting organizational medication-related changes
o Implementing strategies that minimize potential for controlled drug diversion; this includes discrepancy reviews and sustaining a proactive surveillance program
o Analyzing and reporting on medication variance events at both the pharmacy program and organizational levels, and facilitating implementation of process improvements and safety strategies to reduce opportunities for error
o Proactively reviewing medication safety literature to identify risk points/best practices and conduct gap analysis
o Ensuring compliance with legislative requirements, accreditation standards and standards of practice related to safe medication use
o Advancing safe medication practices within the pharmacy department and throughout the organization
o Development, review and/or implementation of medication-related policies at the pharmacy and organizational level
o Reviewing medication safety best practices and evaluating them;
o for implementation in the organization; leading the implementation of best practices; promotes a culture of medication safety
o Collaborate on and assist with developing and maintaining automation, clinical, and operational data in information systems and automation
o Assist with the development and provision of pharmacy HIS-associated training, education and competency validation
o Pharmacist patient care activities as assigned
o Others as assigned
QUALIFICATIONS:
o Current registration with the Ontario College of Pharmacists (Part A) required
o Minimum of 5 years hospital pharmacy experience required (within the last 8-10 years)
o Formal professional development (e.g. courses, certifications, completion of mentorship program, etc.) related to medication safety, medication system design or another relevant topic required
o Formal professional development or training (e.g. courses, certifications, completion of mentorship program, etc.) with hospital pharmacy automation and Cerner HIS build and maintenance required
o Completion of a hospital pharmacy residency preferred
o Demonstrated proficiency and knowledge of safe medication-use practices, ISMP initiatives, practice standards, Accreditation standards, OCP standards, etc.
o Experience in leading medication safety or quality reviews to include process review, recommending changes, and implementation and monitoring of the changes/improvement
o Excellent verbal and written communication skills with a collaborative and engaging management style
o Excellent interpersonal and team building skills
o Demonstrated understanding of clinical practice as it pertains to the integration of clinical information systems and the impact to medication safety
o Successful organizational and time management skills
o Experience in project management, data analysis, and report writing and presentation
o Excellent analytical, critical thinking and decision-making skills
o Consistent and regular attendance required
o Proficiency in both official languages (English & French) will be considered an asset
