WAGE RANGE: $48.720 TO $55.230 PER HOUR
POSITION OBJECTIVE:
Reporting to the Manager, Professional Practice the Advance Practice Nurse - Wound, Ostomy & Continence (WOC) will lead, model, promote and facilitate a best practice approach to WOC care at Niagara Health. The APN-WOC will initially focus on the enactment of the pressure injury/wound care aspects of the Niagara Health Wound Care Action Plan while at the same time developing and initiating an action plan for best practice ostomy and continence care. The APN-WOC will be an expert resource to the Wound, Ostomy and Continence Team and will demonstrate leadership throughout Niagara Health with a focus on providing extraordinary caring through knowledge translation, people/change management expertise, clinical staff development and related policy and procedure development for patients at all sites. The APN-WOC will collaborate and build strong relationships with the WOC team, Directors, Physician partners, Patient Care Managers, Nurse Educators and other nursing and hospital leaders to support and engage nurses and inter-professional team members. The APN-WOC will compile and assist with the reporting of WOC metrics, participate in/conduct research and will participate in product selection for WOC products. The APN-WOC will provide advanced wound, ostomy and continence care through the assessment, treatment and management of wound, ostomy and continence challenges primarily to inpatients. The APN-WOC will work with community providers to ensure the continuum of care for patients. The APN-WOC will provide leadership for various corporate committees, such as the Wound, Ostomy and Continence Steering Committee while collaborating with the reporting Manager, Director of Nursing Professional Practice and Elder Care, Human Resources, Finance, Materials Management and other related teams. Travel between the sites of Niagara Health is required.
QUALIFICATIONS:
o Current registration in good standing with the College of Nurses of Ontario as a Registered Nurse (RN) or Registered Nurse in the Extended Class (RN-EC).
o Master's degree in Nursing, Adult Education or Wound Care.
o Completion of the International Inter-Professional Wound Care course
o Completion of the Nurses Specialized in Wound, Ostomy and Continence - WOC_EP Program achieving the designation of NSWOC
o WOCC ( C) Canadian Nurses Association certification is preferred
o Wound, Ostomy, and Continence specialty certification.
o Five (5) years' RN and/or APN experience in Medical and/or Surgical nursing in an acute care setting.
o Three (3) to five (5) years' clinical experience in an acute or complex care setting with a direct focus on wound care.
o Superior inter-personal and leadership skills including critical thinking, responsiveness, conflict resolution, change management, visionary thinking, organizational skills, effective customer service, decision making skills and problem solving.
o Demonstrated experience in leading teams and enacting a plan of action.
o Excellent communication skills with the ability to relay complex information effectively to patients and health care professionals.
o Demonstrated knowledge translation skills to implement evidence based practices.
o Demonstrated experience with quality improvement strategies and methodologies.
o Demonstrated proficiency with program development and implementation, project and change management and evaluation.
o Committed to excellence in all elements of professional practice, including excellence in patient care processes, measuring outcomes, collaboration and professional growth through continuing education and research utilization.
o Demonstrated ability to be flexible, self-directed and to work independently and as a member of the Professional Practice team.
o Demonstrated ability to manage a large/diversified workload with rapidly changing priorities and challenging deadlines.
o Computer skills in Word, Excel, Access, PowerPoint and Outlook and a variety of audiovisual equipment.
o Must maintain a valid Ontario driver's licence and have access to reliable transportation.
o Demonstrated commitment to the Vision, Mission and Values of Niagara Health.
o Consistent and regular attendance is required.
o Proficiency in both official languages (English and French) will be considered an asset
