WAGE RANGE: $42.69 TO $55.35
POSITION OBJECTIVE:
The Behavioural Supports Ontario (BSO) Case Manager is part of the Geriatrics Program and will support patients, families, and teams across all Niagara Health Sites to ensure effective and person-centred plans of care are developed for persons with cognitive impairments. The BSO strategy involved the implementation of service models within and external to hospitals and long term care homes, which address gaps in the health care system for patients with cognitive impairment who have, or are at risk of responsive behaviours/personal expressions. The strategy was built on the former HNHB LHIN BSO Action Plan (2011) with ongoing evolution to ensure system re-design for this population using quality principles across all sectors of healthcare. The goal of BSO is to improve system-level coordination and integration of all health care sectors, through principles of 'building-on existing', health system re-design, quality principles and building capacity across sectors.
The BSO Case Manager will receive referrals from any member of the interprofessional health care team wherein specialized care plans are required to optimize the patient journey while in hospital and to facilitate timely discharges. The successful candidate will collaborate with the patient, family, unit team, and Geriatrics program colleagues as required as strategies are developed to understand, eliminate, and manage responsive behaviours. She/he/them will facilitate complex case discussions among key partners, partner with key community providers, such as Alzheimer Society staff, Home and Community Care Support Services Case Managers, and Long-Term Care homes to examine opportunities, determine and track system barriers to discharge, elevate themes and barriers to hospital administration and the BSO Strategic Lead.
QUALIFICATIONS:
o Regulated Healthcare Professional with minimum 5 years experience
o Regulated Healthcare Professional in good standing with their respective Regulatory College
o Experience assessing, treating, and developing care plans for complex patients (i.e. geriatrics, mental health, ABI, development delay)
o Experience working in both acute care and long-term care settings
o Specialized skills in assessment of delirium, dementia, and mental health. Knowledge of best practices in these areas.
o Training and experience in teaching Gentle Persuasive Approaches, PIECES
o Training and experience in Montessori techniques
o Excellent interpersonal and team building skills that enable rapport building with stakeholders
o Proven ability to provide coaching and mentoring
o Demonstrates triaging proficiency to identify high-risk indicators and assign priority levels
o Proven ability to deliver under tight deadlines with effective time management skills
o Experience participating in teams to successfully achieve outcomes of a complex nature
o Proven leadership skills that foster engagement and strengths-based outcomes
o Self-starter who can work with limited guidance
o Well-developed analytical and problem-solving skills
o Excellent verbal and written communication skills
o Previous experience implementing a new role or position an asset
o Immunizations as required by NHS, and potentially additional immunizations depending on requirements by partner hospitals or long-term care homes
o Crisis Prevention Intervention Training an asset
o Valid drivers license and access to vehicle to travel across the Niagara Region
o Scheduling flexibility required based on needs of hospitals and other stakeholders as some meetings, patient rounds, etc. may occur early morning or later day. This will result in the need to shift work hours or work over-time
o Proficiency in both official languages (English & French) will be considered an asset
