WAGE RANGE: $66.43 TO $75.87 PER HOUR

POSITION OBJECTIVE:
The Nurse Practitioner (Stroke Specialist) will provide leadership and coordination of services for patients with a diagnosis of stroke across the continuum including: hyperacute, acute and stroke prevention care. The successful candidate will care for patients with stroke in the Emergency Department, Intensive Care, Acute Stroke Unit and the Stroke Prevention Clinic. The Nurse Practitioner (Stroke Specialist) will collaborate and coordinate the transfer of regional acute stroke patients across the system from acute through to rehabilitation and follow up in secondary stroke prevention clinic. The incumbent will develop, implement and evaluate policies and procedures in collaboration with the District Stroke Team to enhance stroke care in Niagara. They will be responsible for supporting education and assistance with onboarding of acute stroke unit/ED/ICU staff in collaboration with the NH educators. The successful candidate will also represent the District Stroke Centre on Central South Regional Stroke initiatives and become part of the District Stroke Centre rotation with services across the care continuum.

QUALIFICATIONS:
o Current registration with the College of Nurses of Ontario - Extended Class
o Masters of Science in Nursing with clinical focus
o Successful completion of a Nurse Practitioner program (Primary Care or Adult) from a recognized university
o Current BCLS
o NIHSS Certification
o Three in the past five years clinical experience in neuroscience/cardiovascular supporting a population of patients with stroke
O Recent Canadian Neurological and STAND experience and proficiency
o Exemplary teaching and leadership abilities and skills
o Proven ability to integrate research into practice
o Self-directed in clinical practice as required with the stroke population
o Demonstrated understanding of the existing processes and mechanisms related to the continuum of stroke care as outlined in the Canadian Stroke best practice recommendations
o Supportive of the principles of patient and family-centered care
o Supportive of organizational and departmental goals and objectives, demonstrating initiative to promote total quality improvement
o Demonstrated commitment to providing quality customer service while dealing with the public (patients and families), as well as various medical and non-medical professionals
o Demonstrated ability to work collaboratively as a member of an interprofessional team
o Evidence of effective organizational, interpersonal, communication, judgment, critical thinking and decision-making skills
o Knowledge of data interpretation and strong computer skills
o Demonstrated commitment to continuing education
o Demonstrated commitment to the mission and values of Niagara Health
o Proficiency in both official languages (English and French) will be considered an asset
o Consistent and regular attendance required
