HOURS OF WORK: 75 HOURS PER PAY PERIOD; INCLUDING SATURDAYS

WAGE RANGE: $66.430 TO $75.870 per hour
POSITION OBJECTIVE:
Reporting to the Director Complex Care and Geriatrics, the GEM Nurse Practitioner (NP) primarily works within the Niagara Health Emergency Departments (ED) and is responsible for comprehensive geriatric assessments; recommendations for care for older adults to optimize function and independence; avoid unnecessary hospital admissions, assisting with implementation of care plans; and assisting with smooth transitions either to acute care or back to the community. The successful candidate will be skilled in developing linkages with community supports that will enable older persons to age at home. Their focus is on older adults who are at risk of suffering adverse events, loss of independence and admission to hospital or long-term care.
The GEM NP will collaborate with ED physicians and staff, the interprofessional team, and community partners.
The GEM NP will have a passion for seniors' health and will play a lead role in the development of system change, advocacy, and senior friendly care environments. The GEM NP will provide education and support to patients, caregivers, and ED staff and physicians related to geriatric syndromes, appropriate age-friendly care, and transitions planning. The GEM NP will provide leadership and promote excellence in patient-centred care through expert practice, coaching and mentoring, education, consultation, and geriatric-focused research.
As a member of the interprofessional team, the GEM NP practices in accordance with Regulated Health Professions Act, Gerontological Nurses' Association (GNA) Standards, Practice Standards set by the College of Nurses of Ontario, and Niagara Health policies and procedures.
QUALIFICATIONS:
o Current registration in good standing as a Registered Nurse Extended Class Primary Health Care or Adult Certification with the College of Nurses of Ontario (CNO)
o Completion of Nurse Practitioner Program and/or Master in Nursing or Master of Science in Nursing
o Must have successfully completed CNO approved controlled substances education
o Must have professional Liability Protection (RNAO or equivalent)
o Completion of an approved geriatrics certification is required within the first year of employment
o CNA Certification in Gerontology preferred
o A minimum of 3 years' experience working as a Nurse Practitioner with adults in acute care, emergency department, ambulatory, complex or Long Term Care settings
o A minimum of 5 years clinical experience as a Registered Nurse adults in acute care, emergency department, ambulatory, complex or Long Term Care settings
o Clinical expertise in gerontological nursing, including providing care to older adults with complex medical and social needs
o Demonstrated clinical leadership skills to organize and coordinate the care of patients in collaboration with their care givers and community partners
o Ability to promote professional education and advanced knowledge through research and the evaluation/quality improvement activities related to the care of patients throughout the continuum
o Evidence of effective organizational, interpersonal, communication, leadership, judgment, critical thinking and decision-making skills
o Demonstrated knowledge of community resources
o Demonstrated commitment to the mission and values of the Niagara Health
o Consistent and regular attendance required
o Proficiency in both official languages (English and French) will be considered an asset
