POSITION OBJECTIVE:
Reporting to the President and CEO, the Development Officer Planned Giving is responsible for ensuring the success of the Niagara Health Foundation’s Planned Giving Program. The Development Officer Planned Giving is also responsible for advancing the mission of Niagara Health Foundation by developing and disseminating Planned Giving materials and fostering strong relationships with financial advisors, friends and donors. By managing a portfolio of prospects, the Development Officer Planned Giving will invite donors to include a gift to Niagara Health Foundation in their will, celebrate donors who have confirmed an estate gift and steward bequests.
RESPONSIBILITIES
o Manages portfolio of planned giving prospects through the donor cycle utilizing principles of relationship-management, and personally conducts a pre-determined number of one-on-one visits with prospects on a yearly basis to secure expectancies
o Assists in the development and implementation of cultivation, solicitation and stewardship strategies for planned giving prospects. This involves personally soliciting prospects for planned gifts (both outright and deferred), as well as providing the strategy and support for others on the Development team to solicit prospects to maximize the opportunities for fundraising success
o Works in collaboration with the Development Team and Foundation staff to set goals and develop plans
o Works with the Development Team to prepare and update Planned Giving materials, including brochures, websites and forms

o Provide personalized support and navigation for Indigenous patients and their families accessing services at Niagara Health

o Facilitate communication with the patient/family and all members of the care team to promote a culturally safe care environment and the delivery of high-quality patient care
o Facilitate the intake and follow-up of Indigenous patient experience concerns/ complaints/ questions and work with the care teams to achieve resolution for patients and families
o Facilitate access to traditional healing practices including, cultural and spiritual practices for patients and families at their request

QUALIFICATIONS:

As a member of the palliative care team, the NP will assess and co-manage patients with palliative care needs; participate in the development and delivery of education to all stakeholders regarding early identification of patients who have a life limiting illness and will participate in or lead other activities designed to improve care for palliative patients across all care settings at Niagara Health. The role requires collaboration with Oncologists, Physicians, Nurses, Allied Healthcare providers, Spiritual Care, Discharge Planners, LHIN partners, Primary Care and other community based palliative care service providers. The objective of the Registered Nurse is to provide safe and competent care through the application of nursing knowledge, the technical aspects of professional practice and the demonstration of compassion, professionalism and respect. QUALIFICATIONS:
o Current registration with the College of Nurses of Ontario
o One year of related clinical experience in the past three years in a neonatal intensive care environment
o Proof of completion of the Perinatal (Neonatal) Intensive Care Course, or willingness to complete within two years
o Current Neonatal Resuscitation certification
o Current BCLS certification
o Demonstrated knowledge and skill in neonatal physical assessment (Neonatal Physical Assessment course)

POSITION OBJECTIVE:
Working to promote clinical excellence, quality patient/family-centered care and high professional standards, the Neonatal Nurse Practitioner will provide comprehensive, holistic, family-centred care to neonatal patients in an acute care setting through the use of advanced skills, knowledge and education in health assessment, diagnoses and management, health promotion and disease prevention. The Neonatal NP works in a collaborative practice model with his/her physician partner(s) and within an interdisciplinary team environment.
