WAGE RANGE: $66.430 TO $75.870 PER HOUR
POSITION OBJECTIVE:
The Nurse Practitioner will work in collaboration with the Niagara Health Hospitalist program. This role is responsible for ongoing assessment and management and planning for patients on the in-patient medical, complex care, and/or corporate ALC units, across the five Niagara Health sites as required. The Nurse Practitioner will report directly to the Program Director. The Nurse Practitioner will provide leadership and promote excellence in patient-centered care through expert practice, education, consultation and research. The successful candidate will provide care with an emphasis on wellness, prevention and disease management through direct patient care, health promotion and education, outreach, advocacy and community development as required.
QUALIFICATIONS:
o Registration in good standing with the College of Nurses of Ontario - Registered Nurse - Extended Class
o Successful completion of a Nurse Practitioner program from a recognized university, obtaining a Master in Nursing or Master of Science in Nursing
o Recent geriatric, primary care or acute care (RN or RN-EC) experience
o Demonstrated skill in program development using a collaborative, stakeholder engaged process
o Demonstrated clinical leadership skills to organize and co-ordinate the care of patients in collaboration with the Hospitalist Team
o Proven success in providing inspirational leadership in a team environment and ability to encourage innovation and creativity amongst team members required
o Ability to be self-directed in clinical practice as required in the areas of assessment, diagnosis and therapeutics
o Ability to promote professional education and advance knowledge through research and the evaluation/quality improvement activities related to the care of patients throughout the continuum
o Applies Niagara Health values and professional practice standards to patient/client care and service
o Demonstrated commitment to patient/client centered care
o Strong analytical and conceptual skills required
o Demonstrated commitment to the mission and values of the Niagara Health System
o Applies knowledge, skills, judgment and competencies required for safe practice as outlined by the College of Nurses of Ontario
o Integrates reflection, critical thinking, education and relevant practical experience in problem solving
o Must be willing to work flexible hours to reflect patient population needs
o Proficiency in French would be an asset
o Consistent and regular attendance required
