POSITION OBJECTIVE:
Niagara Health (NH) is committed to ensuring patient centered transitions comprising of a coordinated, approach from the time of admission through to discharge to the community. NH has a responsibility to the community to optimize the appropriate use of hospital resources ensuring acute care and specialized rehabilitation beds/services are available for patients requiring such levels of care. The Access and Flow Manager will work collaboratively with our Niagara Health clinical teams HNHBB Home and Community Care Support Services, NOHT, and other community partners, to carry out early discharge planning processes that optimizes patient centered care and facilitates a timely and seamless transition from hospital to the community.
The Manager of Access and Flow will coordinate the care pathways of patients from initial visit within the Emergency Department along service care transitions required along the health continuum. This role will support the access and flow of patients to required services throughout the care system by creating a supportive discharge plan. This role will support the day to day operations for NH system flow to ensure timely access to services for our patients and community.

With accountability to the Clinical Program Director of Medicine, Access and Flow, the Manager of Access and Flow will provide operational leadership in the effective transition/discharge of patients from the NH hospital sites to the community. The Access and Flow manager will provide leadership to the NH discharge planning teams and work in collaboration with our clinical manager teams to ensure a positive patient experience.
The Manager of Access and Flow will directly lead identified staff whose function focuses on the scope of discharge planning and transitions in care from hospital. They will be the link connecting the two organizations to embed the philosophy of Home First in all relevant processes supporting seamless and timely transitions between hospital and community.
DAY TO DAY OPERATIONAL MANAGEMENT:
o Plans, manages, and evaluates the delivery of discharge planning processes and transitions in care from NH.
o Accountable for effective team functioning in service and performance standards.
o In collaboration with the multi-disciplinary team, facilitates the flow of patients through their continuum of their care
o Monitors and adjusts caseloads, workload and assignments within the hospital portfolio to maximize efficiency.
o Collaboratively manages complex and difficult patient issues and complaints with timely investigation and follow-up.
o May attend/support staff in Consent & Capacity Review Board hearings, service appeals and legal consultations.
o Acts as a key resource to NH and HNHB H&CCSS staff, other managers, service providers and external stakeholders.
o Ensures that standards are maintained with respect to the assessment of patient needs, planning and coordination, to support patients and their caregivers
o Operationally, interprets NH and goals and performance standards at the team level.
o Reviews the current status of ALC patients as required with the clinical teams and assists with problem-solving to support timely and supportive discharge.
o Coordinates the flow of patients from acute to complex care based on appropriate designation and system capacity
o Monitors and manages resources as required according to established guidelines and priorities.
o Assesses and identifies critical service delivery issues and risks to NH and recommends/implements service improvements as required.
o Evaluates and identifies the hospital and community impact of proposed changes within assigned portfolio. .
o Provides leadership in the development of communication materials to patients and families with respect to admission, hospital stay and discharge.
o Ensures patient services are provided in accordance with NH and HNHB H&CCSS policies, relevant LTC Legislation, Public Hospital Act, other standards and guidelines.
o Creates mechanisms for improved communication, decision-making, and alignment between NH and HNHB H&CCSS team members and the broader health system.
QUALIFICATIONS:
o Regulated Health Care Professional in good standing with respective health professional regulatory body in Ontario is preferred.
o University degree in a health-related field, Master's level preparation an asset
o Minimum 5 years management experience working within a unionized healthcare environment
o Demonstrated recent knowledge of NH/Acute Care and HNHB Home & Community Care priorities, policies, practices and service standards
o Recent experience leading quality improvement initiatives
o Recent experience leading interprofessional team(s) through change at a system level
o Expert knowledge and understanding of legislation governing health care delivery
o Knowledge of direct care/case management models used in community health care
o Demonstrated knowledge of local health and community services
o Working knowledge of financial and statistical reporting
o Effective organizational skills allowing to prioritize and respond to multiple competing demands within defined time lines
o Proficient computer and business skills
o Excellent communication skills with a variety of audiences internally and externally
o Expert collaboration and negotiations skills
o Excellent leadership skills to promote team building and positive organizational culture
o Highly effective conflict resolution skills
o Travel within designated areas to meet with service providers, stakeholders or resolve serious patient issues
o Proficiency in both official languages (English and French) will be considered an asset.
o Consistent and regular attendance required.
o Vaccines (COVID-19 and others) are a new hire requirement of the job unless you have an exemption based on medical or on a ground pursuant to the Ontario Human Rights Code
