WAGE RANGE: $41.390 TO $57.780 PER HOUR
POSITION OBJECTIVE:
Driven by a fundamental passion and commitment to patient/client centered care and inter-professional practice, the successful candidate will be responsible for the planning, implementation and evaluation of clinical education programs at both the corporate and program level. Reporting to the Clinical Manager, Diabetes Management, the Nurse Educator will work collaboratively with Practice, Clinical Managers, the Regional Education Team and other health professionals towards the achievement of NHS educational goals for staff, patients and families.
QUALIFICATIONS:
o Current registration as a Registered Nurse or Registered Nurse Extended Class, in good standing with the College of Nurses of Ontario
o Must have completed a Nursing diploma or BScN with advanced knowledge and background in the adult field of Diabetes as it relates to insulin and oral agent therapy management and patient education.
o Must hold a current Ontario driver's license.
o Must have adult clinic experience in the adjustment of insulin therapy and diabetes related oral agents within the last two (2) years.
o Must have experience in community based adult diabetes education within the last two (2) years.
o Teaching experience in a group forum within a multi-discipline setting is required.
o Teaching experience in an individual counseling forum within a multi-discipline setting is required.
o Demonstrated teaching skills including needs and educational assessment; development of educational plans; defining educational objectives; and the evaluation of education outcomes, in both group and individual counseling is required.
o Must have knowledge of diabetes teaching strategies, resources and evaluation tools for different literacy levels.
o Must have knowledge of the 2018 Clinical Practice Guidelines for Diabetes Management as it relates to the pathophysiology, pharmacological aspects, and current treatment regimes for Type 1 and Type 2 Diabetes and psycho-social dynamics of adapting to the chronic illness for adults.
o Demonstrated understanding and acceptance of the adaptation process required for the chronic illness of Diabetes, its daily self-management regime and long-term complications.
o Must have excellent communication skills with the ability to relay complex information and relate effectively with patients (of all ages and cultural backgrounds), and health care professionals in a calm and professional manner.
o Must have knowledge and demonstrated an understanding in working in a collaborative team approach with physicians and other health care groups located in the community.
o Certification as a Diabetes Educator in accordance to the Diabetes Educator Sector of the Canadian Diabetes
o Association is preferred.
o Evidence of effective customer service, organizational, interpersonal, communication, leadership, judgment, critical thinking and decision-making skills
o Ability to work effectively within a multi-disciplinary team.
o Flexible, self-directed and able to work effectively with minimal supervision.
o Ability to use various audio/visual equipment independently.
o Consistent and regular attendance required.
o Proficiency in both official languages (English and French) will be considered an asset
