WAGE RANGE: $38.02 to $47.83
POSITION OBJECTIVE:
This position is part of the Niagara Diabetes Centre regional program with satellite locations in Niagara Falls, Welland and St Catharines. The incumbent will provide adult/paediatric diabetes management, education and counseling as part of a multi-disciplinary team. Some of the duties include but are not restricted to assessments, determining/educating individuals on insulin to carbohydrate ration as it relates to intensive insulin therapy (multiple insulin injections/insulin pump therapy) through physician consultation and medical directives, meal-plan development/education/evaluation as it relates to Diabetes and any other relevant clinical indices supported by the intervention of a Registered Dietitian. The position is part of a Niagara Diabetes Centre team that works to deliver adult/paediatric Diabetes services within a group, individual counseling or clinic setting. The position will also provide education sessions to community based agencies such as respite organizations, Community and Independent Living Agencies, work place facilities etc. as required. The successful candidate's home site will be the Welland Hospital and must be willing to travel to various satellite locations.
QUALIFICATIONS:
o Current registration with the College of Dietitians of Ontario.
o Must have completed a Baccalaureate in Nutrition and successfully completed a Dietetic Internship Program.
o Certification as a Diabetes Educator in accordance to the Diabetes Educator Sector of the Canadian Diabetes Association is preferred.
o Must have recent experience in adult diabetes care and nutrition counseling as it relates to intensive insulin therapy within the last two (2) years.
o Clinical and educational experience in the community setting with a diabetes focus is preferred.
o Teaching experience in a group setting as well as on an individual basis, in a multi-discipline setting.
o Knowledge of diabetes teaching strategies, resources and evaluation tools for different literacy levels.
o Must be able to use various audio/visual equipment independently.
o Must have knowledge of the pathophysiology, pharmacological aspects, current treatment regimes for Type 1, Type 2 and Gestational Diabetes.
o Demonstrated understanding and acceptance of the psycho-social adaptation process required for the chronic illness of diabetes, its daily self-management regime and long-term complications.
o Must have excellent communication skills with the ability to relay complex information and relate effectively with patients (of all ages and cultural backgrounds), and health care professionals in a calm and professional manner.
o Must have knowledge and demonstrated an understanding in working in a collaborative team approach with physicians and other health care groups located out in the community.
o Evidence of effective customer service, organizational, interpersonal, communication, leadership, judgment, critical thinking and decision-making skills.
o Ability to work effectively within a multi-disciplinary team.
o Flexible, self-directed and able to work effectively with minimal supervision.
o Must hold a current driver's license.
o Proficiency in both official languages (English and French) will be considered an asset.
o Consistent and regular attendance required.
o Vaccines (COVID-19 and others) are a new hire requirement of the job unless you have an exemption based on medical or on a ground pursuant to the Ontario Human Rights Code
