HOURS OF WORK: 7.5 HOURS PER PAY PERIOD; MON-FRI; ON CALL AS NEEDED

WAGE RANGE: 33.230 TO 47.570 per hour

POSITION OBJECTIVE:
Working to promote nursing excellence, quality patient/client-centered care and high professional standards, The home hemodialysis nurse is responsible for meeting the needs of a unique patient population by providing education and ongoing support and care of patients who chose to dialyze independently and/or with a helper in their homes. This position provides an opportunity to utilize enhanced clinical assessment skills, provide patient training and education to renal patients and their families while promoting self-management of their chronic disease. The incumbent will perform all other relevant duties assigned to ensure efficient operation of the home teaching unit. The successful candidate's home site will be the Niagara Falls Dialysis Satellite Unit and must be willing to work at two or more sites. Hours of work may vary according to patient availability to train for home dialysis. In the event there are no qualified applicants, this will be used as a training opportunity.

QUALIFICATIONS:
o Current registration with the College of Nurses of Ontario
o Minimum 3 years hemodialysis experience
o Minimum 1 year experience in adult teaching with knowledge of adult teaching principles
o Experience in the development, implementation and maintenance of policies and procedures
o Experience with data management in relation to patient statistics and renal network
o Experience in the development, implementation and maintenance of policies and procedures
o Demonstrated excellent interpersonal and communication skills, both verbal and written
o Demonstrated ability to be self-directed and independent, as well as work with the interdisciplinary team members, patients and families
o Demonstrated effective conflict resolution skills
o Demonstrated excellent organizational skills with the ability to be flexible and effectively work in a o busy environment with changing priorities and deadlines
o Demonstrated leadership in order to provide, facilitate and promote quality patient care
o Demonstrated initiative in improving work skills and processes
o Demonstrated excellent patient assessment skills
o Demonstrated commitment to professional development
o Must possess a valid Ontario driver's license and be able to drive to home visits
o On call coverage as required
o Proficiency in both official languages (English and French) will be considered an asset
