WAGE RANGE: $36.917 TO $47.046 PER HOUR
POSITION OBJECTIVE:
Reporting to the Manager of Access and Flow, the incumbent will be responsible for facilitating patient flow from pre-admission through discharge and into the community. The successful candidate's home site will be the Site listed above, however hours of work may be required at other sites within the Niagara Health System. Weekend and holiday hours of work may be expected.
QUALIFICATIONS:
o Bachelor's degree in a healthcare/related field with active membership in a health or health related professional college
o Must be a member in good standing of respective regulatory College (COTO, CNO, OCSWSSW)
o Current license/registration in related field
o Three years of hospital clinical experience
o Two years recent experience in case management, utilization review/management understanding of Alternate Level of Care (ALC,) patient care flow process and of community services
o Experience in collaborative care within hospital and community setting
o Demonstrated knowledge of medical terminology, ability to understand patient care plans, pathways and treatment plans.
o Demonstrated knowledge of coordinating and leading discharge planning/destination meetings with the interdisciplinary care teams and patients and their families.
o Evidence of effective organizational, interpersonal, communication, leadership, judgment and decision-making skills
o Applies Niagara Health values and professional practice standards to patient/client care and service
o Demonstrated commitment to patient/client centred care
o Applies skills by providing, facilitating and promoting quality patient care within an inter-professional team
o Demonstrated commitment to patient and family education and the ability to promote health and wellness
o Integrates education, critical thinking, and relevant practical experience in problem solving
o Consistent and regular attendance mandatory
o Proficiency in both official languages (English and French) will be considered an asset
