WAGE RANGE: $50.590 TO $57.670 PER HOUR
POSITION OBJECTIVE:
Reporting to the Director Nursing Professional Practice and Elder Care, the Nurse-Led Outreach Team (NLOT) Nurse Practitioner (NP) works in partnership with the Niagara Region NP, Long Term Care Home leadership and staff, LTC home attending Physicians and NPs, and Niagara Health staff and physicians, to assist in reducing non-urgent transfers to the region's Emergency Departments, support smooth transitions from hospital to LTC homes, and help build capacity in the nursing staff in the LTC homes while increasing awareness of LTC home needs for hospital staff.
The NLOT NP will work collaboratively to enhance return to LTC for those residents wherein the ED visit was unavoidable and where the LTC home staff require additional support and mentorship to meet the care needs of the resident(s). The NP will provide: assessment, intervention, monitoring and follow-up for acute and chronic illness; education and support to care givers, family and LTC Home staff, others as required/requested; and care coordination. The Nurse Practitioner will provide leadership and promote excellence in patient-centred nursing care through expert practice, education, consultation and research.
As a member of the Inter-professional team, the NLOT NP practices in accordance with Regulated Health Professions Act, Gerontological Nurses Association (GNA) Standards, The Long Term Care Homes Act, the Practice Standards set by the College of Nurses of Ontario and the standards set out for the Nurse Practitioner. The NLOT implements and maintains an effective program(s) to reduce acute care admissions to hospital from the long-term care (LTC) sector, with an emphasis on wellness and disease prevention, health promotion and education, mentoring of staff, outreach, advocacy and community development. Program development and direction will reflect community needs, hospital needs, LHIN and MOHLTC priorities.
QUALIFICATIONS:
o Current registration in good standing as a Registered Nurse Extended Class Primary Health Care or Adult Certification with the College of Nurses of Ontario (CNO)
o Master's degree in Nursing or related field
o Must have successfully completed CNO approved controlled substances education
o Must have professional Liability Protection (RNAO or equivalent)
o A minimum of 3 years' experience working as a Nurse Practitioner in Acute Care or Long Term Care settings
o A minimum of 5 years clinical experience as a Registered Nurse in Acute Care or Long Term Care settings
o Clinical expertise in gerontology
o Clinical expertise including but not limited to: IV initiation, suturing, venipuncture, wound care, end of life care planning.
o Demonstrated clinical leadership skills to organize and coordinate the care of patients in collaboration with their care givers
o Ability to promote professional education and advanced knowledge through research and the evaluation/quality improvement activities related to the care of patients throughout the continuum.
o Evidence of effective organizational, interpersonal, communication, leadership, judgment, critical thinking and decision-making skills.
o Demonstrated commitment to the mission and values of Niagara Health.
o Consistent and regular attendance required.
o Valid Ontario Driver's license and access to car required
