HOURS OF WORK: 75 HOURS PER PAY PERIOD; MONDAY - FRIDAY
DURATION: 6 MONTHS

WAGE RANGE: $33.900 to $48.530 PER HOUR
POSITION OBJECTIVE:
Reporting to the Clinical Manager, Professional Practice, the successful candidate will be part of the NiagaraHealth@Home program, specifically will play a lead role for; identifying and enrolling appropriate patients; communication with service provider organization partners; and communication and monitoring patient and family experience while enrolled in the program.
The successful candidate will collaborate with Niagara Health Discharge Planners, Charge Nurses, service provider organization partners, care teams, and Home and Community Care - Care Coordinators, to coordinate efficient patient flow activities and safe care transitions to home. Some of the duties include but are not restricted to providing an overview of the NiagaraHealth@Home program to patients, families and care teams; performing home care specific needs assessments; developing in partnership with care teams a 72-hour home plan of care; participating in case conferences at regular scheduled intervals; collaborating with service provider organization partners to ensure care plans are met; liaising with members of the interprofessional team (both within the hospital and externally); participating in program evaluations and daily care huddles.
The successful candidate will be a positive change agent who plays a pivotal role in the success of this initiative as the duties and responsibilities continue to evolve based on program growth and the ongoing complex needs of our patients and families. Travel may be required within the Niagara Region and between Niagara Health sites.
QUALIFICATIONS:
o Current registration in good standing with the College of Nurses of Ontario as a Registered Nurse (RN).
o Minimum of five years' RN experience in medical and/or surgical acute care, complex care, or community based direct care.
o Strong understanding and experience with NH flow activities and discharge planning process.
o Knowledge of community and government resources and relevant legislation, including Excellent Care for All Act and Home First philosophy.
o Excellent communication skills with the ability to relay complex information effectively to patients, families, and health care professionals.
o Demonstrated knowledge translation skills to implement evidence based practices.
o Superior inter-personal and leadership skills including critical thinking, responsiveness, conflict resolution, visionary thinking, organizational skills, effective customer service, decision-making skills and problem solving.
o Experience with quality improvement, change management, and evaluation is an asset.
o Committed to excellence in all elements of professional practice, including excellence in patient care processes, measuring outcomes, collaboration and professional growth through continuing education and research utilization.
o Demonstrated ability to be flexible, self-directed and to work independently and as a member of the Professional Practice team.
o Demonstrated ability to manage a large/diversified workload with rapidly changing priorities and challenging deadlines.
o Must have proficient computer skills in Word, Excel, Access, PowerPoint and Outlook and a variety of audiovisual equipment.
o Must maintain a valid Ontario driver's licence and have access to reliable transportation.
o Demonstrated commitment to the Vision, Mission and Values of Niagara Health.
o Consistent and regular attendance is required.
o Proficency in both official languages will be considered an asset
