WAGE RANGE: $50.590 TO $57.670
POSITION OBJECTIVE:
Working to promote clinical excellence, quality patient/client-centered care and high professional standards, the Nurse Practitioner in Palliative Care will provide palliative care consultation services and leadership in the implementation of Niagara Health System's strategic priorities for palliative care consistent with the Declaration of Partnership and Commitment to Action (2011) and adopted by the province of Ontario. The ideal candidate will demonstrate a high level of collaboration, critical thinking and expertise in clinical decision making and system development. As a member of the palliative care consultation team, the NP will assess and co-manage patients with complex palliative care needs; participate in the development and delivery of education to all stakeholders regarding early identification of patients who have a life limiting illness, require non-complex palliative care and will participate in or lead other activities designed to improve care for palliative patients across all care settings at the Niagara Health System. The role requires collaboration with Hospitalists, Specialists, Nurses, Allied Healthcare providers, Spiritual Care, Discharge Planners, CCAC, Primary Care and other community based palliative care service providers. The successful candidate must be willing to travel to all sites.
QUALIFICATIONS:
o Current registration with the College of Nurses of Ontario in the Extended Class.
o Masters Degree in Nursing required.
o Certificate in Palliative Care CHPCN(c).
o Current BCLS.
o Minimum three (3) years of related clinical experience in Palliative Care.
o Applies knowledge, skills and judgment to meet nursing practice standards as outlined by the College of Nurses.
o Demonstrated leadership, critical thinking, interpersonal and communication skills, both written and verbal.
o Demonstrated ability to work in a consultative role with members of an inter-professional team.
o Demonstrated ability to be flexible and adapt positively to constantly changing clinical situations.
o Demonstrated ability to lead organizational change.
o Demonstrated ability to implement, monitor and respond to quality indicators.
o Compliance with confidential requirements under the NHS Privacy Policy.
o Working knowledge of Microsoft Office applications and the ability to learn about other software/computer applications needed to carry out the role.
o Proficiency in both official languages (English and French) will be considered an asset.
o Consistent attendance and the ability to work independently with optimal time management skills are required.
