WAGE RANGE: 32.860 to 47.040 per hour
POSITION OBJECTIVE:
Working to promote nursing excellence, quality patient/client-centered care and high professional standards, the successful candidate will coordinate the care related to vascular access for patients approaching Hemodialysis and those patients currently receiving Hemodialysis treatment. The incumbent will act as a liaison between Vascular Surgery, Interventional Radiology and Nephrology service to ensure patients have functioning vascular access and coordinate, implement and evaluate the vascular access treatment plan. In addition the incumbent will collaborate in the development of vascular access policies and procedures and participate in research activities as appropriate. The incumbent will lead the implementation of quality initiatives established by Ontario Renal Network. The successful candidate's home site will be the site listed above and must be willing to work at two or more sites.
QUALIFICATIONS:
o Current registration with the College of Nurses of Ontario.
o Current BCLS training.
o Current Certification in advanced nursing skills related to Hemodialysis
o Canadian Nurses Association Certification in Nephrology preferred.
o 5 Years clinical experience in the past 7 years in hemodialysis
o Current knowledge of the pathophysiology of CKD (Chronic Kidney Disease), pharmacological aspects of CKD, dietary and fluid management and psychosocial dynamics of adaptation to chronic illness.
o Excellent assessment skills and demonstrated ability to develop plan of care.
o Demonstrated professionalism and knowledge of nursing practice standards as outlined by the College of Nurses related professional practice expectations, and legislation requirements.
o Working knowledge of computers preferred.
o Supportive of organizational and departmental goals and objectives, demonstrating initiative to promote total quality improvement.
o Demonstrated commitment to providing quality customer service while dealing with the public (patients and families), as well as various medical and non-medical professionals.
o Ability to work effectively within an interdisciplinary team.
o Evidence of effective organizational, interpersonal, communication, leadership, judgement, critical thinking and decision-making skills.
o Flexible, self-directed, and able to work effectively with minimal supervision.
o Demonstrated commitment to continuing education.
o Consistent and regular attendance required.
